Fold Raises $6 Million to Simplify Value-Based Care
Abhijit Gupta and Ram Sahasranam, two seasoned veterans in the health tech space, have a wealth of experience in launching successful startups. Their previous venture, Praxify, was sold to Athenahealth, but they’re now focusing on value-based care with their new startup, Fold.
A Solution for Value-Based Care
Fold is an operating system designed to alleviate administrative burdens associated with value-based care. This approach ties provider reimbursement to patient outcomes, which can be challenging to implement without the right infrastructure. Gupta and Sahasranam have seen firsthand how existing tech systems hinder the transition to value-based care.
Challenges in Value-Based Care
During their time working on Praxify and other health startups, Gupta and Sahasranam identified several obstacles that prevented providers from adopting value-based care. One of the main issues was the lack of data tools that could accurately track patient data and support care decisions. Many primary care physicians also experience burnout due to heavy administrative workloads.
A Unified Platform for Value-Based Care
Fold’s operating system addresses these challenges by providing a unified platform that integrates with electronic health records (EHRs) and offers modular tools for customization. This allows providers to automate much of the work required for value-based care, including:
- Care gap closure: Identifying patients who require follow-up care
- Transitional care management: Coordinating care during transitions between healthcare settings
- Annual wellness visit completions: Ensuring timely completion of annual wellness visits
- High-risk patient engagement: Engaging with high-risk patients to prevent hospitalizations
- Attribution growth and protection: Managing attribution and preventing waste
Streamlining Administrative Tasks
Fold’s care team enablement suite gathers all patient data, task management, omni-channel communications, and scheduling to coordinate care teams. The platform also includes a white-labeled patient web and app portal that allows patients to stay in touch with their care teams.
Success Stories
One early adopter of Fold is a primary care clinic that had high burnout rates among staff due to its fragmented tech ecosystem. By switching to Fold’s unified platform, the clinic was able to reduce administrative tasks and allow providers more time for patient care.
Next Steps for Fold
With its new funding, Fold will focus on developing additional features targeting primary care consumers and reducing administrative inefficiencies.
